Abstract : Background Type 2 diabetes mellitus (T2DM) has become a serious public health issue in Saudi Arabia. However, there are few studies on dietary adherence among patients with T2DM. In this study, we estimated the degree of dietary adherence among patients with T2DM and examined factors associated with high dietary adherence. Objectives The primary objectives of this study were to assess the degree of dietary adherence among T2DM patients attending primary health care centers (PHCCs) at Makkah city and to determine the factors associated with high dietary adherence. Methodology A cross-sectional study was performed between May 2016 and April 2017 in the PHCCs of Makkah city. We randomly selected eight PHCCs for our analysis. From each selected PHCC we choose 25 male and 25 female patients by systematic sampling. We interviewed the study participants using a questionnaire that had three sections: socio-demographic characteristics, the validated Perceived Dietary Adherence Questionnaire (PDAQ), and the validated General Practice Physical Activity Questionnaire (GPPAQ). We used the t-test and the one-way analysis of variance (ANOVA) to examine differences in dietary adherence. We used multiple linear regression to identify factors associated with dietary adherence. Results A total of 345 patients agreed to participate. Their mean age was 55.20 years (SD= 12.22 years), and the patients had diabetes for an average of 9.67 years (SD= 7.01 years). The mean dietary adherence, as measured by the PDAQ, was 25.38 (SD= 6.91) and the frequency of high dietary adherence (≥ mean PDAQ) was 47.25%. Significantly higher dietary adherence was found in females (= 3.01; 95%-CI 1.52 - 4.51; p< 0.001), 40-60 year old patients (β= 3.72; 95%-CI 1.38 - 6.05; p= 0.002), >60 year old patients (β= 2.67; 95%-CI 0.24- 5.10; p= 0.031), patients with >10,000 Saudi Riyals monthly income (β= 2.14; 95%-CI 0.13-4.15; p= 0.037), patients with moderate physical activity (β= 1.82, 95%-CI 0.06-3.37; p= 0.037) and patients with family support (β= 2.27; 95%-CI 0.80-3.75; p= 0.003). Significantly lower dietary adherence was found in obese patients (β= -4.65; 95%-CI -6.47(-)-2.83; p< 0.001) and patients with glycated hemoglobin level ≥7mg/dl (β= -3.04; 95%-CI -4.57(-)-1.51; p< 0.001). Conclusion and Recommendations Our study showed low degree of dietary adherence among type 2 diabetic patients in Makkah city. Interventions to enhance dietary adherence should focus on obese adult male patients with low monthly income.
