Abstract : Background: Black garlic or aged black garlic is a type of fermented garlic which has been used in Oriental countries for a long time because of various biological properties of garlic derivatives. Objective: The current study aimed to investigate the effect of oral administration of aqueous extract of Aged Black Garlic Extract (ABGE) for 7 weeks on hepatocarcinogenesis induced to male Wistar rats by Diethylnitrosoamine (DEN) and Carbon tetrachloride (CCl4). In addition to determine the ABGE content of S-allyl-cysteine (SAC). Methods: Fourty five male Wistar rats were randomly divided into five groups (n = 9) as follows: Group A (control); Group B (DEN-group) received a single intraperitoneal injection of DEN (200 mg/Kg b.wt.), two weeks later received a dose of CCl4 (2 mL/Kg b.wt.); Groups C, D and E received the same carcinogenesis protocol as group B plus ABGE oral doses 150, 300 and 600 mg/Kg/d, respectively for 7 weeks. Results: The results showed that oral administration of ABGE significantly decreased the elevated serum levels of hepatic enzymes (aspartate aminotransferase (AST), alanine aminotransferase (ALT)), total bilirubin (TBIL) compared to Group B (DEN-group). Levels of antioxidant enzyme glutathione reductase (GR) and Trolox Equivalent Antioxidant Capacity (TEAC) were significantly increased as compared to Group B (DEN-group). Levels of serum pro-inflammatory cytokines (Tumor necrosis factor-α (TNF-α) and Interleukin-2 (IL-2)) were significantly decreased as compared to Group B, while levels of serum anti-inflammatory cytokine Interleukin-4 (IL-4) were significantly increased as compared to Group B. Histopathological and immunohistochemistry examination of liver and spleen sections of rats administered ABGE showed alleviation of histological degenerative changes caused by DEN and CCl4. Conclusion: The results suggest that ABGE has remarkable hepatoprotective and antioxidant effects and improve immunity function in DEN-initiated and CCl4-promoted hepatocarcinogenesis model in rats.
