Background: Decision-making is a vital element for the nurses’ professional performance. It leads to achieve the expected finding and rising patients' satisfaction since nurses have the most continuous contact with patients. However, the nurses' view and experience on the uncertainty and interaction with the multitude of factors which may facilitate or inhibit their decisions abilities has rarely been studied. Key words: Nursing decision-making process (DMP), factors influencing nurses' decisions, naturalistic decision-making approach, managerial decision, and clinical decision.
Aim: The aim of this study is to explore nurses' perception about DMP they use when making decisions, and the various factors that influenced this process within their actual work environment.
Method: This study was conducted by using a qualitative research design. A purposive sample of 52 nurses (27 nurse managers, 25 staff nurses) from general and critical wards in two major hospitals from ministry of health in Hail city. Nurses were recruited to participate in 8 focus group interviews. A semi-structured, open-ended questions interviews were used to elicit participants’ responses. The data were analyzed by using thematic analysis.
Results: Key findings had been emerged and categorized into five major themes which include; 1) The current situation of nursing DMP that included the assessment of the situation, following the policies of the hospital, and involving the others; 2) The influence of personal nursing factors on nurses’ DMP which included nurse's knowledge, experience, beliefs and cultural awareness, physical status, psychological status, and nurse's gender; 3) The influence of organizational factors on nurses’ DMP which included the availability of the resources, organizational support and approachability, the workload, the availability of educational program, the availability of monitoring program, and the consistency of policies application & unity of rules and regulations; 4) The influence of patient related factors on nurses’ DMP which included patient physical and psychological condition, patient socio-cultural influences, patients and family rights & preferences, and patient safety; 5) The influence of environmental factors on nurses’ DMP which included the physical layout of the unit and room features.
Conclusion: The research findings revealed the current situation of the DMP among nurses and nurse managers, and indicated numerous factors that may facilitate or inhibit the quality of nursing care process and decision making ability in MOH hospitals. The significance of this study is the comprehensive illustration of the decision-making influential factors at patient, personal, environmental and organizational level. This study has presented a comprehensive view of nurses’ perceptions of their role in the decision-making which has supported the previous studies. As a result, this study has utilized a framework which could explain the nature of the nurses DMP, and emphasized the imperative need to strengthen nurse's supportive environment for enhancing the quality of nursing care and improving the nurse's ability to make the right decisions to achieve the desired outcomes for patients and health organizations. |
