Abstract : Metabolomics is the study of metabolites that aims to characterize and quantify all small compounds in complex biological samples. Metabolite profiling has become a powerful approach that has been widely adopted for clinical diagnostics. Metabolomics studies can be divided into two distinct approaches: non-targeted and targeted metabolomics. The goal of this study was to evaluate the metabolomics profile in obese and obese diabetic patients as an early predictor of diabetes with obese patients. The current study was conducted on fifty-four volunteers. Participants were mainly 35–70 years of age and classified to four groups according to BMI and HbA1c which are control, obese, diabetic and obese/diabetic. Serum was subjected for analysis of lipid profile, leptin, thyroid hormones (FT3, FT4 and STH), and metabolomics (lipidomic and proteomics). Serum samples were analyzed for metabolomics using GCTOF-MS and LCQTOF-MS to investigate the polar and nonpolar metabolites. Highly polar metabolites were identified by GCTOF-MS whereas negative and positive modes for nonpolar metabolites (lipids) were analyzed using LCQTOF-MS. The results obtained demonstrated that there are a significant increase in saturated fatty acid (myristic acid C14:0) in the diabetic with (P˂0.05) as compared with control groups. In addition, a significant increase in unsaturated fatty acid (palmitoleic acid) in obese and diabetic as compared with control with fold change (0.34) and (P˂0.05). Whereas a significantly increased in myristic acid and phenylalanine were observed in obese and diabetic with (P=0.04) &(P=0.01) as compared with control respectively. A significant correlation found (P˂0.05) and a correlation coefficient of (-0.652), indicating that there is a strong relationship between Leptin and FT3. It was concluded that metabolomics of lipid intermediate was considered as a good index of metabolic syndrome and can be taken into consideration for follow up and treatment.
