Abstract : Background: Sore throat is a common problem during childhood and is usually the result of bacterial or viral infection. Group A Streptococcus (GAS) is the most common bacterial cause; up to thirty percent (30%) of children with a sore throat. There are many viruses that cause sore throat of an upper respiratory infection named “common cold”. Other viruses include Influenza, Enterovirus, Adenovirus and Epstein-Barr virus (the cause of mononucleosis). Objectives: Estimating the clinical decision rules (CDRs) predicting biostatistics value of Centor criteria/Rapid Antigen Detection Test (RADT) to find the positive and negative predictive value, sensitivity, specificity versus gold standard method. Method: Diagnostic prospective cohort study involving fifty patients (50) with 15 years or less conducted at primary health care clinic at National Guard Specialized Polyclinics in Jeddah, Two throat swabs samples were collected from patient: one for laboratory culture of GAS, the other for RADT. Data collection were being filled according to physician (Appendix D). Results: The present study finds a significant relation between total Centor criteria score and indication of antibiotic at p-value < 0.000, sensitivity (50%), specificity (100%), the positive predictive value and negative predictive value (100%) & (88.88%) respectively, significant relation between culture result and RADT bedside patients at p-value < 0.001, sensitivity (90%), specificity (100%), the positive predictive value and negative predictive value (100%) & (97.56%) respectively. Conclusions: RADT tend to increase the sensitivity of diagnosis bacterial pharyngitis in comparing to the presence of signs and symptoms in Centor criteria score.
