Abstract : Rheumatoid arthritis (RA) is a chronic inflammatory of autoimmune disease characterized by destruction of articular cartilage and bone destruction leading to ultimately disability. The most common chronic autoimmune disease affects approximately 1% of the population worldwide. It is characterized by persistent synovitis, pain, swelling, and progressive destruction of the small joints of the hands and feet, accompanied by a functional disability. Although the ethology and pathogenesis of RA remain unknown, it is generally considered to be an autoimmune disease in which pathogenic T cells play an important role. Th17, Th1 has pro-inflammatory role in RA. The main importance Th17 cytokines are IL-17, IL-21, IL-22, and to lesser extent IL-6 and TNF-α. The IL-17 cytokine family consists of six members, IL-17 A, IL-17B, IL-17C, IL-17D, IL-17E, and IL-17F. The specific polarizing cytokines needed by Th17 differentiation and propagation is TGF-β, IL-6, and IL-23. The current study aims to identify the levels of Th17 cytokines profiles (IL-17, IL-21, IL-22, TGF-β, IL-6, and IL-23) in Saudi rheumatoid arthritis for first time. in addition to Th17 cytokines levels comparison between healthy and RA patient and Th17 cytokines profile detection in both genders who were 69 females and 9 males. We analysed these cytokine profiles in 96 blood samples including 78 patients and 18 healthy control by commercially available ELISA kits. The study finds out that cytokines, IL-6, IL-17, IL-22, IL-23, TGF-b, TNF-α, and TNF-RN, largely recorded positive or higher levels of cytokines. This implies how cytokines proves to be crucial as they help in regulating inflammation, destruction and autoimmunity destruction in patient associated with rheumatoid arthritis. In addition, differentiation in the levels between the cytokines profiles and rheumatoid arthritis in patient may predicted the quality of life expected among the patients.
